Creating a PATH: Improving Pediatric to Adult Transitions in Healthcare

Objectives and Disclosures

Participants will be able to:

  1. Appraise current pediatric to adult healthcare transition practice in context of the 6 core elements of transition.
  2. Outline the benefits of an organized transition process including improved adherence and medical outcomes, patient/provider satisfaction and medical equity.
  3. Design system processes and apply resources for supporting providers and young adults during the transition process to improve their patients' experience.
