Objectives and Disclosures
Participants will be able to:
- Appraise current pediatric to adult healthcare transition practice in context of the 6 core elements of transition.
- Outline the benefits of an organized transition process including improved adherence and medical outcomes, patient/provider satisfaction and medical equity.
- Design system processes and apply resources for supporting providers and young adults during the transition process to improve their patients' experience.
