How to Make an Appointment
You can make an appointment with Seattle Children’s Craniofacial Center without a doctor’s referral. But information from your child’s doctor can help us schedule the right team members for your first appointment. Please ask your doctor to fax us your child’s clinic notes with a New Appointment Request Form.
Out-of-Region and International Support
We offer dedicated support for families who live outside the Northwest. If you live outside of Alaska, Idaho, Montana, Washington, please call 206-987-0184 or email us for more information.
We will ask you to send us your child's medical records, any CT scans and photos of your child's face and head.
How to Refer a Patient
If you are a provider, fax any clinic notes (such as head circumference and a description of head shape) along with a New Appointment Request Form (PDF) (DOC) to 206-985-3121 or 866-985-3121 (toll-free). See Refer a Patient for more information.
