Patient Information

Choosing a pediatrician for your child is a big decision. We want to make sure all new families feel welcome and comfortable, whether you are expecting a new baby or transferring from another practice.

We work hard to establish a bond with each child and family from the initial appointment and during every future visit. We’re here to answer your questions and provide you with the best care possible.

We accept most private insurers and accept qualified patients on the Washington State Apple Health/DSHS program. Please visit our insurance page to see which plans we accept.

To Do

Patient forms will be available on MyChart or may be filled out on an iPad in the waiting area before the appointment.

If you're transferring from another practice, please also fill out this form to authorize the release of pertinent medical information to our office:

We will provide a copy in clinic if you are unable to print ahead of your next visit.
