Provider-to-provider communication
- For non-urgent clinical questions, please call the clinic at 206-987-5173, option 5 to leave a non-urgent message for an audiologist.
- For co-management questions related to an existing problem for a shared patient, please send an InBasket message via EpicCare Link.
- If you would like to have one of our Audiologists provide in-person or virtual education with your clinic, please contact our Physician Relations team to coordinate.
How to Refer a Patient
We encourage use of our New Appointment Request Form (PDF or DOC) or submitting a referral via EpicCare Link. Please complete these forms in their entirety to ensure timely processing of your patient’s referral. If your clinic uses its own referral form, please ensure all fields listed in the NARF are included in your documentation.

Additional referral information
Audiology sees patients through age 21 for suspected or known hearing loss and provides diagnostic services, including behavioral and physiologic assessments, and fitting and management of hearing devices.
What Your Patients Can Expect
- Families will receive a text message when their referral has been entered into the system and is ready to schedule.
- Typically, outbound scheduling calls only occur for urgent referrals – please encourage families to call to schedule.
- Established patients with hearing devices can use MyChart to schedule Ear Mold/Hearing Aid Service.
- Comprehensive audiology services are offered at all locations, with the exception of sedated BAER testing, which is only available at Seattle Children's Hospital main campus.
