Short-Form: Consent and Assent Forms

When using the short form consent process for any studies that have been initiated in the Click IRB system, please use the appropriate translated Short Form Consent Form and Short Form Assent Form (if necessary) from the lists below or in the Click IRB Library. Studies that were processed in the legacy IRB system should use the legacy forms.

CLICK-Initiated Short-Form Consent Forms (Old Regs)

The following forms are to be used for studies initiated in the Click IRB system in which the study approval was received before January 21, 2019. The English version of the Short Form Consent and Short Form Assent can also be found in the Click IRB Library.

CLICK-Initiated Short-Form Consent Forms (New Regs)

The following forms are to be used for studies initiated in the Click IRB system in which the study approval was received on or after January 21, 2019. The English version of the Short Form Consent and Short Form Assent can also be found in the Click IRB Library.

HIPAA Form: Standalone Document for Reliance

When using the standalone HIPAA form, it should not be altered, except where it is allowed within the form. If editable fields within a translated standalone HIPAA form are changed, a new certified translation must be obtained.
