Consent and Assent Templates

New studies approved on or after July 17, 2026, must use Seattle Children’s current versions of consent and assent templates. Current versions are available under the Templates tab of the Click IRB Library. Previous versions are provided below for reference.

ONLY use these templates for studies approved in Click IRB between December 5, 2022, and July 16, 2026. Below is the latest version of the Click template to be used.

Short-Forms

When using the short form consent process, the appropriate translated Short Form Consent Form and Short Form Assent Form should be used.

New studies approved on or after July 17, 2026, must use Seattle Children’s current versions of the Short Form Consent and Short Form Assent. Short Form Consent Forms

Standalone HIPAA Forms

Seattle Children’s incorporates HIPAA authorization language into the informed consent documentation whenever possible. However, there may be circumstances where a standalone HIPAA form must be used (e.g., if an external IRB requires it).

When using the standalone HIPAA form, it should not be altered, except where it is allowed within the form. If editable fields within a translated standalone HIPAA form are changed, a new certified translation must be obtained.

New studies approved on or after July 17, 2026, must use Seattle Children’s current version of the standalone HIPAA form. Current versions of the standalone HIPAA form (including translations) are available under the Templates tab of the Click IRB Library. Previous versions are provided below for reference.
