What is the goal of the study?

A multicenter randomized controlled trial will be conducted in children and adolescents with cystic fibrosis (CF) who are on CF modulators and experiencing worsening of respiratory symptoms. The aim is to assess the efficacy and safety of two approaches to treatment: immediate antibiotics versus tailored therapy. Participants in this main cohort will be enrolled for 12 months with quarterly research visits to collect clinical data. A subset of participants who are preschoolers or non-modulator eligible will also be enrolled and followed until first exacerbation. Participants will report worsening of symptoms to the research team and will be contacted to assess eligibility for the study treatment. If treated, participants will be closely followed to determine clinical response and need for change in therapy. Sub-studies related to microbiology, inflammation, and outcome measures will be optional.
