About Case Management

  • Español PDF

We want to be sure your child’s hospital stay and discharge are as smooth as possible. To do this, we:

  • Strive for clear communication between the healthcare team and you, with you at the center.
  • Work with your providers to create, review and change the plan of care to meet your child’s unique needs.
  • Connect you with resources so your child can get back to your community.

The Case Management team can support you and your child in these ways:

Rounds, coordination, communication

Your child’s registered nurse case manager (RNCM) will:

  • Participate in healthcare team meetings and family meetings
  • Ensure we make the best use of resources to meet your child’s needs
  • Work with staff nurses and the entire healthcare team on a plan of care
  • Arrange care for your child to leave the hospital

Home nursing

For care at home, the RNCM can arrange public health nursing, visiting nursing, hourly/private-duty nursing or other nursing care.

Home care services

Our team can help you get services and supplies for home care, including:

  • Special feeding equipment and supplies
  • Home infusions
  • Respiratory equipment and supplies
  • Medical equipment like wheelchairs or hospital beds
  • Training to take care of your child

Medical transfers

The RNCM can arrange air and ground transportation for your child.
