School Services

Refer a Patient

Patients at Seattle Children’s can access school services through a referral from their providers. Any care team member can refer a patient for school services. Our team works with patients who receive inpatient and/or outpatient treatment. If you are unsure if a referral is appropriate, please email or call us.

Epic

The primary way to refer a patient for school services is to place an order through Epic.

Release of Information

Before we can contact a patient’s school to release, obtain or exchange information, we will need a signed authorization form. See Health Information Management for more information.
