Health Information Management and Medical Records Requests

Seattle Children’s MyChart is free, secure and has been expanded to give you even more access to your child’s healthcare team and medical record. If you need more information than you can find in MyChart, you can easily request copies of your Seattle Children’s medical records and radiology images.

How to Request Medical Records

  1. Complete a form: Patients or parents/legal guardians must fill out the authorization form.
  2. Submit the form: Send the form to Seattle Children’s Health Information Management:

  3. Receive records: 
    • Once your request is received, a response will be provided within 15 business days.
    • Please note that our office does not allow for in-person pickups or drop-offs.
    • Electronic image delivery is preferred and the fastest method to receive copies of medical records.

Delivery options

  • Compact disc

    Electronic records (except for radiology images) are password protected. To have the password emailed to you, please provide your email address on the authorization form. If no email address is provided, the password will be mailed separately to the postal address listed on the authorization form.

  • Electronic: MyChart or email

    Please note, there are file size restrictions when sending records through MyChart and E-Mail. If the file size is too large, the HII department will contact you to determine the best way for you to receive records.

    If email is selected, the patient or requestor understands and accepts the potential risks of email communication. You must provide an email address to receive medical records in this format.

How to Request Radiology Images

  • Seattle Children’s MyChart

    With an active Seattle Children’s Hospital MyChart account users can download JPEG (reference quality) and DICOM (diagnostic quality) images via their Radiology results in MyChart. See more information and instructions.

  • Nuance Power Share

    Through the free Nuance PowerShare network, you can securely view and download your child's radiology images. You have 45 days to access them from any internet-connected device.

    How it works
    Accessing your child’s radiology exams
    • Once your account is set up, log in and click the "Images" tab.
    • Viewing exams: To view the images, click on the radiology image you want to see
    • Downloading exams: To download JPEG exams to your computer, select the Export tab and click the ‘Export Image to JPEG/PNG’ icon in the upper left.
    • Download DICOM files: From the ‘Images’ tab, click the checkbox next to the image thumbnail and then choose Download DICOM from the menu on the right.

    In person pickups are only available for radiology images on CD. If selected on the Authorization to Release/Obtain/Exchange Patient Health Information radiology images may be picked up at the following locations.

    Please note, that a staff member will call to alert you when the images are ready for pick up.

Revocation of Authorization Form

You have the right to cancel your Authorization to Release Patient Health Information. To do so, complete the Revocation of Authorization Form (PDF) and send it to Seattle Children’s Health Information Management.

Access Medical Records Online 

Sign up for Seattle Children’s MyChart to see outpatient and inpatient records online. Visit the Seattle Children's MyChart - Login Page to access your medical records from your clinic visits or hospital stays at Seattle Children’s Hospital. Some medical records aren't directly available in MyChart, but you can still request a formal copy of your health record by following these steps:

How to request your records in MyChart:

  1. Log in to Seattle Children's MyChart - Login Page.
  2. Go to the Patient Menu and select Sharing Hub.
  3. Click "Request formal copy of health record."
  4. Fill out all the required fields, review your request, and then submit it.
