Thank you. We have received your request to no longer receive fundraising materials from Seattle Children’s. Please allow at least 2 weeks for this communication to be honored, and we sincerely apologize if there is already something in the mail to you. Again, choosing to opt out does not impact your care at Seattle Children’s Hospital in any way.

Important note for households with more than one child seen at Seattle Children’s: Please complete the opt-out form for each individual child.
