Thank you for referring your patient to the PAL Plus Program! A local behavioral health specialist will contact the family within 3 business days to set up an initial phone screening.

Patient care update will be communicated as follows:

  • Primary care providers – Updates will be faxed directly to your office throughout treatment. As part of the patient’s care team, communication is covered by HIPAA, section 45 CFR 164.506; no additional release of patient information is required
  • School-based staff – A release form from the student/family will be required prior to sharing any patient care updates.
