Event and Project Supply Request Form

The Guild Association keeps an inventory of supplies to support guild fundraising efforts. These items are available on a first-come, first-served basis.

We recommend that you submit your requests at least two weeks before your event.

Items will be available for pickup at the front reception desk of the Guild Association. In some cases, small items may be mailed to guilds outside the Seattle area.

All materials must be returned within one week of your event's completion.
