Conference Fees
Registrations will be accepted in the order received. Space is limited. The conference fee includes catering, parking validation, instructional materials and CME credit or certificate of attendance.

Registration Information
All participants must pre-register. There will be no on-site registration. Online/mail-in and faxed registrations must be received by July 12, 2013.

Cancellation policy
You may cancel registration through July 12, 2013, and receive a full refund minus a $25 cancellation fee. No refunds will be issued after July 12, 2013.
