Please complete one of our family information forms. You can e-mail completed forms to us at intake@seattlechildrens.org or mail them to us at Psychiatry Intake Office, MS W-3636, PO Box 5371, Seattle, WA 98105-0371.
- For children 5 years old and younger: Early Childhood Patient and Family Information Form (DOC).
- For children older than 5 years: Patient and Family Information Form (DOC).
- Information on your outpatient mental health evaluation (PDF).
- Bring test results from clinics other than Children’s.
- Bring any assistive devices your child uses that are related to the reason for your visit.

- Please arrive on time. If you are more than 10 minutes late, we may need to reschedule your visit.
- If you cannot keep your appointment, please give as much notice as possible.
- If you miss an appointment or cancel with less than 48 hours notice 3 times, we may not be able to continue to serve you and your child at our clinic.
- Therapy visits last about 45 to 50 minutes.
- Visits about medication usually last about 20 to 50 minutes.
