Prepare for Your Visit to Psychiatry and Behavioral Medicine

Please complete one of our family information forms. You can e-mail completed forms to us at intake@seattlechildrens.org or mail them to us at Psychiatry Intake Office, MS W-3636, PO Box 5371, Seattle, WA 98105-0371.

  • For children 5 years old and younger: Early Childhood Patient and Family Information Form (DOC).
  • For children older than 5 years: Patient and Family Information Form (DOC).
  • Information on your outpatient mental health evaluation (PDF).
  • Bring test results from clinics other than Children’s.
  • Bring any assistive devices your child uses that are related to the reason for your visit.
  • For information on visiting Children’s clinics, please see Your Child's Clinic Visit.
  • Please arrive on time. If you are more than 10 minutes late, we may need to reschedule your visit.
  • If you cannot keep your appointment, please give as much notice as possible.
  • If you miss an appointment or cancel with less than 48 hours notice 3 times, we may not be able to continue to serve you and your child at our clinic.
  • Therapy visits last about 45 to 50 minutes.
  • Visits about medication usually last about 20 to 50 minutes.
