  • Before your child's appointment, we ask that you and your child (if they are able) each answer written questions about your child's pain. Your answers help us get a complete history of your child's pain problems. They also help us as we work with you and your child to develop an effective treatment plan. We have questionnaires for children (PDF), teens (PDF) and parents (PDF).
  • Please complete the right questionnaires and mail them to us before your child's appointment. If there is not enough time to mail the questionnaires, bring them with you to your child's appointment.
