  • Before your child's visit, we ask you and your child (if they can) to answer written questions. Your answers help us get a complete history of your child's pain and make a treatment plan that works. We have questions for children and teens (PDF) and parents (PDF).
  • Please answer the questions, and mail your answers to us before your child's visit. If there is not enough time to mail your answers, bring them with you to your child's visit.
