  • We need your child's medical records from clinics, hospitals or other healthcare providers outside of Children's. At least two weeks before your child's appointment, please either mail records to us or make sure your child's primary care provider has sent us the records. Examples of the records we need are X-rays or other images that we have requested, radiology reports, laboratory reports and documents related to your child's condition, such as your family's history with the condition or a note stating that a parent has been diagnosed with a genetic condition.
  • We have your child's records from Children's in our files, so there is no need to send those to us.
