How to Make an Appointment

You can make an appointment with Seattle Children’s Cancer and Blood Disorders Center without a doctor’s referral. To help us prepare for your first appointment and coordinate your child’s care, ask your doctor to fax us your child’s clinic notes with a New Appointment Request Form.

How to Refer a Patient

If you are a provider, fax a New Appointment Request Form (PDF) (DOC) to 206-985-3121 or 866-985-3121 (toll-free). See Refer a Patient for more information.
